We set aside your appointment time just for you. Our office runs on time and you will be promptly seated in the treatment room at the time of your appointment. This process allows you the convenience to fill everything out prior to arriving for your appointment. Your session will time out due to security, so select a time you can fully complete the process. The page may offer a print option, however, due to security, once the print button is selected, all previously entered information will disappear. It is your option to fill out the forms online to save you some time in our office, or to fill them out in our office.

We encourage our patients to fill out their registration forms on our HIPAA secure website, prior to arriving in our office.

  • A form of identification
  • Bring your referral card
  • Pharmacy information
  • A list of current medications you may be taking
  • Any recent x-rays if available, and any dental insurance information

Prior to your first appointment, be sure to indicate if you have any of the following medical conditions:

 

  • A history of aritifical joints of valves (knee, hip replacement, etc.)
  • previous history of bacterial endocarditis  or any artificial prosthesis.

PAYMENT ARRANGMENTS

After your initial exam with Dr. Alan Farber, Dr. Vasiliki Batalias, or Dr. Chris Chondrogiannis, your diagnosis will be determined. Moving forward with preventive care and treatment can be a financial concern for you and we at Farber Center For Periodontics & Dental Implants understand.

Upon return to our office for your no charge conference, the Doctor will review your recommended dental treatment plan and one of our team members will review financial options that best suit your needs. We have several payment options available to ensure you get the dental care you need.

For your convenience, we accept cash, personal checks, most major credit cards, extended payment plans, andinsurance. Flexible financing options are available through Lending Club and Care Credit. With these plans, you will be able to select monthly payments that fit within your budget, turning the oral health you have always wanted into reality.

INSURANCE MAY COVER ALL OR SOME OF YOUR TREATMENT, DEPENDING ON YOUR POLICY

Our insurance coordinator is here to help you in obtaining the full benefits you are entitled to under
your dental insurance plan. There are hundreds of dental insurance plans and some companies change
policies and guidelines weekly. Therefore we encourage you to bring all your insurance benefit
information, including your booklet, forms and insurance card to your appointment, and to keep us
informed of any change to your insurance.

What should I do in a Dental emergency?

Contact our office without delay. Our Periodontists will advise you as to the best course of action. While some concerns can wait until the following day, some must be treated immediately. With cases of injury, the steps you take in the first few minutes can affect long-term outcomes. For instance, a tooth that has been knocked out of its socket can be re-implanted if handled properly and addressed within the first 30
minutes to an hour after the trauma.
